How You Heard

Search for the person’s first or last name. Or, if you know it, enter in the ID of the person. The ID could be the Life Shotz Member Number like “M1234” or a Distributor Number like “1808123456.” You may have seen one of these numbers on a business card, email, or website.

Your Information
Create Login: Enter a valid email address where you want us to send your email, and then create a password for the new Life Shotz account.
Contact Information: Enter your first and last name. Your middle initial is optional. Your date of birth is required for enrollment. Provide at least one phone number.
Spouse Information: Adding a spouse to the account is optional. If a spouse is added, the date of birth is not required.
Tax Information: We require a valid Federal ID or Social Security
Number for Tax purposes. If you are a business, provide the company name and contact
person.
Federal ID and Social Security Numbers
are securily stored and only used for Tax purposes.
Shipping Address: Please note that UPS Ground will not ship to
a PO Box.
Billing Address: If the billing address is different than the shipping address, uncheck the box and a separate billing address fields will be provided.
Email Subscription: This is how our Brand Reps stay in touch with product announcements. Unsubscribing to this free service will cause you to miss out on vital information.
